Appointment Protocols

The Basics

Please bring the following to your eye examination appointment:

  • Your health card
  • An up-to-date list of your medications
  • Your third-party insurance information, if applicable
  • Bring any glasses you currently wear, including prescription sunglasses
  • If you are a contact lens wearer, please arrive wearing your glasses
  • Every patient may have their pupils dilated so please bring some sort of sunglasses if you own them

OUR PROTOCOLS FOR THE DAY OF YOUR APPOINTMENT

Due to the close proximity of our care and that the majority of our patients are over the age of 65, we still recommend you wear a mask; however, it is no longer mandatory.

Patient safety is our #1 priority. We are taking these precautions to provide a safe space for your visit. If you are experiencing any Covid-like symptoms please re-schedule your appointment.

If you are a new patient or have not been seen in the clinic since 2019, we ask that you complete the patient form on our website www.waterdownoptometric.ca/new-patient. Please be sure to fill this out 2-3 days prior to you scheduled appointment.

We have a strict 24 hour cancellation policy. If you do not attend your appointment or cancel with fewer than 24 hours’ notice, you will be automatically charged a fee of $60 for the missed appointment.

To ensure that our doctors get the best view of a patient’s retina OPTOS retinal imaging is a part of all eye exams. The fee for a standard comprehensive eye exam is $193; it is $216 for all contact lens wearers.

The retinal imaging fee for all OHIP-covered patients is $66.00 for adults, $49.00 for youths. Contact lens assessments are NOT covered by OHIP and the fee for those is $77.
